VILLAGE HEALTH CENTER (CLINIC)
The Village Health Center is dedicated to serving individuals currently experiencing homelessness, individuals who were recently homeless and individuals who are at-risk of homelessness. As a Federally Qualified Health Center (FQHC), we see patients regardless of ability to pay or insurance status.

- Behavioral Health (mental health, substance use disorder, and medication-assisted treatment services, including individual and group therapy)

Registration:
The registration process can be completed online (via DocuSign) or you can print, complete and fax documentation to the health center to facilitate the process. If you are uncertain of your insurance coverage or if you need coverage, our Outreach & Enrollment Specialist will reach out to offer assistance. If you need assistance changing your PCP to our health center, we can provide this support as well.

Please Note: VHC’s Dental Registration packet must be filled out onsite before your first dental appointment/service. Please allow enough time before your appointment to complete this documentation.

Located at the Father Joe’s Villages downtown campus in the Paul Mirabile Center (where the public lunch line is daily).
HOURS OF OPERATION
- Open Monday through Friday from 8:30am to 4:45pm.
- Extended hours on Thursdays from 5:30pm to 8:45pm.
- Closed for lunch daily from 11:45am to 12:30pm.
- Closed on holidays.

AFTER HOURS SUPPORT
The Village Health Center has a 24/7 Nurse Triage line that patients may call for assistance when the health center is closed. The 24/7 line can be accessed through our main phone number at 619-645-6405.
PHARMACY
We are happy to send prescriptions to our patient’s pharmacy of choice. Our primary partners include Allen Pharmacy and CVS.
