Requesting Medical Records
To request your Village Health Center medical record be sent directly to you, your non-VHC physician or other authorized person, please review the options below.

Before requesting your medical records, please consider signing up for our patient portal at VHCMyChart , where you can retrieve a variety of documents from your medical record. You can print and share these documents to meet your needs. Most medical records are available online at no charge through our patient portal.

Request Records through our Release of Information Department
Medical Records Request by Paper Form or In Person:
If you don’t have a MyChart account, you can submit a paper request for your medical records following the steps below. Please review both pages of the form thoroughly prior to signing, and be aware that whoever you give your health information to must be responsible for securing it.
You may ask that the authorization form be mailed to you by contacting the Release of Information Department at 619-645-6405.
- Download, complete and sign the form below. Be sure to complete all items containing an asterisk (*). All items need to be completed prior to fulfilling the request.
